One of our social workers visited you!!! During our social work evaluation, we assessed you or your loved one’s current condition to better understand your needs and provide information on community resources and connect them with them.

One of the most important referrals is the LONG TERM CARE PROGRAM. Depending on when you are located you can contact the helpline.

 The Long Term Care program is a medicaid waiver program that allows you to receive either services at home, day care center or financial assistance to reside in an Assisted Living Facility (ALF). At home you can receive assistance with Activities of Daily life (ADL) including personal care, meal preparation or meal delivery, incontinence supplies, homemaking and personal response system among other services.

THERE IS A WAITLIST and is not immediately available. You may be eligible even if you don’t qualify for regular medicaid. The income guideline in 2022 for an individual is $2,523 and for a couple $5,046. There is also a limit with assets but if you don’t meet the financial criteria you can still receive services through the Community Care for Elderly (CCE)- sliding scale fee and assistance from an elderly law attorney for medicaid planning. 

The process is to be referred to the LONG TERM CARE PROGRAM though one of the agencies mentioned above. After the initial referral, AFA or ADRC will contact you to complete a phone assessment (WHICH IS THE MOST IMPORTANT PART OF THE REFERRAL PROCESS) to determine the need and priority. After the assessment is completed you will be placed in a waitlist and will be released based on funding and priority.

Another important referral is TRANSPORTATION– Our social worker provided information about transportation options including insurance or homebound transportation. The benefits include that you are picked up from your home and can go anywhere not just medical appoinments. There is a small fee based on your county. The main disadvantage verbalized by other members is lack of reliability and call in advance.

MEALS ON WHEELS is another one of the most important referrals. This program also has a waitlist and is based on income. Most meals are frozen and do not meet specific dietary needs. You can call them directly based on your location.

Please remember that all the referrals have a WAITLIST, and the Social Worker doesn’t know when you are going to be released from it.  If you have any other questions about the referrals
